Help for Oregon Healthcare Professionals

We provide two programs to help healthcare professionals and their patients after harm events—and create a safe space for learning to reduce the risk of patient harm.

Despite everyone’s best intentions, patient harm sometimes occurs. Our programs support a better response to patient harm and encourage a culture of safer care.

I want to talk with a patient or family member about a medical harm event.

EDR may be able to assist

If someone experiences serious harm or dies while under your care, Early Discussion and Resolution (EDR) connects you with the patient or their family to talk confidentially about the harm that occurred.

This conversation is a chance to acknowledge their experience and share how you plan to prevent similar harm going forward.

I want to help improve patient safety by sharing what I’ve learned from an adverse event.

Your shared experience can make a difference

Through our Patient Safety Reporting Program (PSRP), healthcare facilities throughout Oregon can learn from each other’s experiences by voluntarily providing information about serious patient harm— or near misses— as well as their strategies for preventing future events. We then analyze these details to better understand how and why the harm occurred and share the broader lessons learned. We believe healthcare facilities throughout Oregon should be able to learn from each other’s experiences with patient harm so we can work together to improve overall care.
