Our Programs

Despite everyone’s best intentions, patient harm sometimes occurs. Through the Oregon Patient Safety Commission’s programs, we offer a constructive space for healthcare facilities, providers, and patients to learn from what happened—and make care safer going forward.

Our programs support continuous, system-wide learning and improvement because this work is never finished—and it shouldn’t be done in isolation.
